Beth Israel Medical Center surgeons use newer breast reconstruction techniques that rebuild the breasts in the most natural way possible.

Advances in breast reconstruction techniques have dramatically improved the options for breast cancer treatment. At Beth Israel Medical Center, our renowned reconstructive plastic surgeons are using the DIEP (deep inferior epigastric perforator) flap technique in which the surgeon removes skin and fat from the abdomen and transplants it to the mastectomy site. Unlike older procedures, the DIEP flap doesn't require the removal of abdominal muscle when forming the breast mound. The skin and fat along with the small blood vessels that go into them are detached and reconnected in the breast area so that the tissues can live up there. In addition to an improvement in the abdominal contour, another benefit of DIEP flap surgery is faster recovery and less abdominal discomfort.

FDA recently cleared for marketing a new test called the MammaPrint that can predict the odds that an early stage breast cancer will metastasize in five to ten years. Using a biopsy sample taken from the tumor, the test analyzes the activity of 70 genes which affect whether or not an early stage breast cancer will spread. Based on this, it rates the patient as "high risk" or "low risk" for metastases. MammaPrint is intended for women who are under 61 years old with Stage I or II disease who are lymph node-negative and whose tumors are no larger than 5 cm.

MammaPrint's reliability differs a great deal depending on which risk category it places the woman in. When the test designates the woman as "low risk," there is 90% probability that she will not get metastatic disease at 5 years. But when the test designates the woman as "high risk," there is only a 25% chance that she will actually develop distant metastases at 5 years. It is important for women and their physicians to understand this difference, and to keep the test's limitations in mind.

It is also important to keep in mind that the MammaPrint test is not intended to diagnose breast cancer, or to select the type of therapy, or to predict the woman's response to it. It is a prognostic marker only, to be used along with other clinico-pathological factors.

MammaPrint is currently available overseas. The test's developer, the Dutch company Agendia, is exploring ways to make the test available in the U.S.

My mother died of metastasized breast cancer in February of 2001 after valiantly struggling with the disease for five years. After her death, in the years that followed, a large part of coping with the emptiness in my life was my art. I first started what I now call the "Fiberglass Busts Project" driven by subconscious motives I wouldn't understand until much later. It began from a need to create something out of the emptiness in my life, a time consuming project to fill my days with errands and give me a reason to get out of bed in the morning. Earlier in 2001, I had gotten it into my head that I wanted to create a sculpture using fiberglass, a media I had never worked with before. In the past, I had made countless three dimensional pieces out of paper mache, a favorite medium of mine. It is an easy, cheap and forgiving material to work with, but ultimately the end result always felt temporary. Of all the paper mache sculptures I had created in college, not one of them survived in a U-Haul. For what I was preparing to do, that subconscious motivation was telling me I needed to go stronger, more durable. permanent.
